An Aboriginal woman died from burns to up to 40 per cent of her body after being bathed by staff from Live Better.A disability care provider has apologised to the family of an Aboriginal woman who died after suffering burns to up to 40 per cent of her body in a bath.

During a one-day hearing on Tuesday, the court was told Ms Lucas had multiple disabilities and health conditions, including Cornelia de Lange syndrome. "Ms Lucas was seen moving and vocalising in a manner that was different to her usual behaviour," Ms Munro told the court. "The Live Better employees were there to care for Ms Lucas, but through the failures, which Live Better accepts, contributed to the death of Ms Lucas."

Counsel for both Live Better and the commission provided submissions to Justice Raper about the basis on which a penalty could be determined for the contraventions.Tracy Mackey steps down just days before an independent report into a scandal exposed by Four Corners in which children with autism were unlawfully restrained at an NDIS-funded program.
